AOHC Opening Session: C. O. Sappington Memorial Lecture "The Swamp: The Everglades, Florida, and the Politics of Paradise"

This lecture is named for Clarence Olds Sappington, MD, DrPH, (1889-1949), a noted consultant in occupational diseases and industrial hygiene, who was the first American to hold a DrPH degree which he received from Harvard School of Public Health in 1924. This memorial lecture series, presented annually at AOHC, was created in 1954 to honor this pioneer in the field of occupational medicine and serve as a forum from which to address major issues in the field.

Michael Grunwald, senior national correspondent for Time magazine and author of The Swamp: The Everglades, Florida, and the Politics of Paradise, is this year’s Sappington Lecturer. Mr. Grunwald will speak on how the poor health of the Everglades parallels and impacts the health of the residents in the area.

In The Swamp, Michael Grunwald—winner of the George Polk Award for national reporting and the Worth Bingham Award for investigative reporting—tells the dramatic story of the Everglades, which was once America's last frontier, and is now the focus of the largest environmental restoration project in the history of the earth. “It is the story of a remarkable swath of real estate and the remarkable people it has attracted,” he writes, “from the aboriginals who created the continent's first permanent settlement in the Everglades, to the U.S. soldiers who fought a futile war of ethnic cleansing in the Everglades, to the dreams and schemers who have tried to settle, drain, tame, develop, sell, preserve and restore the Everglades. The purpose of this session is to present a systematic approach to the critical appraisal of the occupational and environmental literature.  Relevant journal article will be evaluated.  The elements to be examined include the study hypothesis, study design, selection of the study population, data collection methods, statistical analysis, and the internal and external validity of an article.  Methods learned in this course can be used as a framework to critically evaluate medical research articles in other disciplines, as well as one’s own research.  This session will allow discussions of methodological, regulatory, research, ethical, or health issues raised by the article presented and also allow the audience to review statistical methods.

Optimizing the Use of Functional Capacity Evaluations

The first part of the session will provide a logical framework to use in determining what type of functional testing to order for a given evaluee and given situation, followed by a description of what criteria should be sought in choosing an evaluator. The goal here is to help clinicians maximize the probability of obtaining useful results from FCEs by carefully defining their needs and arranging for any customization of testing that may be required beforehand.  There will then be a brief description (to the effect that this is possible due to time constraints) of FCE components – i.e. those tests, examination techniques, etc that are used to obtain data. A brief summary of Information regarding the evidence-basis for some of these will be provided as well.  The last part of the lecture will discuss the principles that the physical or occupational therapist should use in interpreting the overall clinical “validity” of FCE results using a terminology that, while new, can be applied to existing FCE models. The lecture ends with suggestions regarding wording that the physician can use to communicate the results of FCE testing, especially if demonstrated abilities are inconsistent and clinically unsubstantiated.  Additional fee required.  Seating is limited: Pre-registration is required.

Work-related asthma, which includes occupational asthma and work-exacerbated asthma, is the most common form of occupational lung disease in developed countries. Because of the associated morbidity, disability and costs related to this disorder, it is important to evaluate and manage workers with occupational asthma.  In this session, attendees will learn about defining occupational asthma/work-related asthma and identifying causes and approaches to the diagnosis of occupational asthma.  Guidance will also be given for administrative issues in the workplace for workers with occupational asthma including environmental/engineering controls, respiratory protection, workplace restrictions and maximal medical improvement determination and finally, information will be provided for overall management of the worker with occupational asthma.  The target audience for this session is occupational health care providers with clinical interest in pulmonary diseases, as well as those interested in impairment ratings for lung disease, reducing environmental/workplace exposures.  This session is sponsored by the Occupational and Environmental Lung Disorder Committee.

More than one million United States service members have returned from serving in Afghanistan, Iraq or both. Less than half of these men and women are seeking health care from the VA, therefore there is a significant likelihood that they are or will seek their care from health care providers in their communities and/or at their workplaces. Many of the issues facing these Veterans are unique to this population and others are rarely seen in occupational medical practices. Estimates are that more than 25% of these Veterans have concerns about one or more deployment-related exposures. This session will acquaint participants with what the exposures of concern are for these Veterans, some of the latest science regarding these exposures, problems of reintegration being faced by these returning service members, a signature injury of the current war, and some practical information about how to talk about exposure risks. A specialty program of the VA that is heavily involved in exposure issues of Veterans will also be presented.

Hands-on-orthopedics is what should be learned before graduating medical school as far as a pertinent and complete musculoskeletal examination of the 4 or 5 diagnoses that are most common for each body area.  The session is targeted towards physicians who deal with musculoskeletal ailments on a regular basis and wish to adjust and tune-up their physical examination skills.  Approximately 4 or 5 of the most common diagnoses for each body area will be discussed as to pathophysiology, physical exam diagnoses and current treatment.  This is a hands-on course and attendees will be expected to perform certain physical exam maneuvers on the person sitting next to them or a person of choice.  The first half will be upper extremities.  The second half will address the lower extremities.
